The Impact of Recurrent Clostridium difficile Infection on Patients’ Prevention Behaviors

OBJECTIVE
To determine the impact of recurrent Clostridium difficile infection (RCDI) on patient behaviors following illness.
METHODSUsing a computer algorithm, we searched the electronic medical records of 7 Chicago-area hospitals to identify patients with RCDI (2 episodes of CDI within 15 to 56 days of each other). RCDI was validated by medical record review. Patients were asked to complete a telephone survey. The survey included questions regarding general health, social isolation, symptom severity, emotional distress, and prevention behaviors.
RESULTSIn total, 119 patients completed the survey (32%). On average, respondents were 57.4 years old (standard deviation, 16.8); 57% were white, and ~50% reported hospitalization for CDI. At the time of their most recent illness, patients rated their diarrhea as high severity (58.5%) and their exhaustion as extreme (30.7%). Respondents indicated that they were very worried about getting sick again (41.5%) and about infecting others (31%). Almost 50% said that they have washed their hands more frequently (47%) and have increased their use of soap and water (45%) since their illness. Some of these patients (22%–32%) reported eating out less, avoiding certain medications and public areas, and increasing probiotic use. Most behavioral changes were unrelated to disease severity.
CONCLUSIONHaving had RCDI appears to increase prevention-related behaviors in some patients. While some behaviors are appropriate (eg, handwashing), others are not supported by evidence of decreased risk and may negatively impact patient quality of life. Providers should discuss appropriate prevention behaviors with their patients and should clarify that other behaviors (eg, eating out less) will not affect their risk of future illness.

Objectives: The aim of this study was to develop a decision support tool to assess the potential benefits and costs of new healthcare interventions.
Methods: The Canadian Partnership Against Cancer (CPAC) commissioned the development of a Cancer Risk Management Model (CRMM)—a computer microsimulation model that simulates individual lives one at a time, from birth to death, taking account of Canadian demographic and labor force characteristics, risk factor exposures, and health histories. Information from all the simulated lives is combined to produce aggregate measures of health outcomes for the population or for particular subpopulations.
Results: The CRMM can project the population health and economic impacts of cancer control programs in Canada and the impacts of major risk factors, cancer prevention, and screening programs and new cancer treatments on population health and costs to the healthcare system. It estimates both the direct costs of medical care, as well as lost earnings and impacts on tax revenues. The lung and colorectal modules are available through the CPAC Web site (www.cancerview.ca/cancerrriskmanagement) to registered users where structured scenarios can be explored for their projected impacts. Advanced users will be able to specify new scenarios or change existing modules by varying input parameters or by accessing open source code. Model development is now being extended to cervical and breast cancers.

Linear birefringence measurements of dilute and semi-dilute polyisobutylene solutions following flow through a disordered fixed fibre bed of 2.47% solids volume fraction provide both transient and steady measurements of chain deformation. Our results indicate that the flexible polyisobutylene polymers undergo a large conformation change, stretching in the direction of the average flow. This occurs even though the average flow in the bed is a plug flow which would not cause any polymer stretch by itself. The polymer stretch or conformation change increases with the number of chain interactions with bed fibres and ultimately reaches a steady-state value that can be correlated with the pore-size Deborah number (i.e. a characteristic polymer relaxation time divided by a characteristic flow time in the bed pore). Large changes in the polymer conformation are noted for values of the Deborah number, De > 5. In addition, the time to steady state scales with the characteristic flow time within a pore over a large range of Deborah numbers. The pressure drop across the fibre bed was also measured simultaneously with the birefringence measurement and was found to be directly proportional to the birefringence throughout the range of De investigated. Thus, we show empirically, for the first time, that chain elongation, which produces normal stress anisotropy within the fluid, is directly responsible for the increased flow resistance. These findings are then analysed in the light of recent theories for the response of polymer molecules in fixed bed flow fields (Shaqfeh & Koch 1992). It is shown that our results are consistent with the interpretation that these flows are stochastic strong flows, which create an apparent ‘coil-stretch’ transition. After extending the theory of Shaqfeh & Koch to account for the specifics in the experiments, including the bed geometry and statistics as well as the polydispersity of the polymer solutions, it is shown that the theory can predict most of the experimental results both qualitatively and quantitatively.

The residual stress induced in a Pt-aluminide bond coat formed on a single-crystal superalloy has been measured. The stresses arise because of the thermal expansion misfit with the substrate on cooling from the manufacturing temperature. Since the lattice parameters are unknown, the interpretation of diffraction measurements is problematic, and the “wafer” curvature method has been applied. This method required that the substrate thickness be systematically reduced by mechanical thinning. It was also required that curvature measurements be made with the bond coat present as well as after it had been removed by thinning. This approach revealed that the bond coat is in residual tension, 140MPa, consistent with its thermal expansion coefficient, relative to that for the substrate.

The mechanism controlling the cyclic failure of a commercial thermal barrier system has been investigated. The system comprises an electron-beam physical vapor deposited (EB-PVD) yttria-stabilized zirconia thermal barrier coating (TBC), deposited on a (Ni Pt) Al bond coating. The thermally grown oxide (TGO) layer that forms between the TBC and bond coat at high temperature is unstable with respect to out of plane displacement, provided initial perturbations are present. With cyclic thermal exposure, the TGO displaces into the bond coat at periodic interfacial sites. The out-of-plane displacements induce strains above the TGO, normal to the interface, that cause cracking. The cracks nucleate either within the TBC layer or at the TBC/TGO interface, and extend laterally until they coalesce with cracks from other sites and coating failure occurs by large scale buckling. The TGO displacements are accommodated by visco-plastic deformation of the underlying bond coat, and are driven by a lateral component of the growth strain in the TGO. The susceptibility of the TGO to out-of-plane displacement depends critically upon the initial morphology of the metal/oxide interface. The observed material responses are compared with predictions of a ‘ratcheting’ model.

Teaching Basic EMTs Endotracheal Intubation: Can Basic EMTs Discriminate between Endotracheal and Esophageal Intubation?

Hypothesis:
Advanced airway intervention techniques are being considered for use by basic emergency medical technicians (EMTs). It was hypothesized that basic EMTs would be able to discriminate reliably between intratracheal and esophageal endotracheal tube, placement in a mannequin model.
Design:An airway mannequin with a closed chest cavity was intubated randomly either esophageally or tracheally, and the cuff was inflated. A stethoscope, bag ventilator, and laryngoscope were available next to the mannequin. Placement was assessed by auscultation or direct visualization at the discretion of the EMT. A blinded investigator graded the student.
Setting:A classroom in a large, urban medical center.
Participants:Subjects were basic EMTs who volunteered to take part after the conclusion of a six-hour endotracheal intubation training course.
Results:Thirty-three subjects were tested. Seventeen of 18 (94%) tracheal intubations and 11 of 15 (73%) esophageal intubations were identified correctly. Only 72% of the students listened to the epigastrium, 81% listened to the lungs, and 85% attempted ventilation. The 10 students who visualized the cords discovered all five esophageal intubations. The 23 students who did not visualize the cords missed four and found six esophageal intubations.
Conclusion:Basic EMTs had difficulty assessing endotracheal tube placement in a mannequin model. The 27% miss rate for identifying esophageal intubations suggests that basic EMTs will require additional training for safe field use of any airway that requires assessment of tube placement.

The yield and composition of milk from Friesian cows grazing either perennial ryegrass (Lolium perenne) (G, ten cows) or white clover (Trifolium repens) (C, nine cows) were evaluated between d 21 and 129 post partum. The two forages, of similar digestible energy content, were the sole source of nutrients and were offered ad lib. (exp. 1). Digestion and flow at the duodenum were measured on 13 occasions in early lactation with comparable cows fitted with rumen and duodenal cannulae and grazing similar forages (exp. 2). The gross milk yield (22·2, G; 25·0, C, kg/d P < 0·05) and the yield of protein (0·66, G; 0·77, C, kg/d, P < 0·01) were higher, but the protein content was similar and the fat content lower for cows fed C compared with G. Cows fed G were heavier at the beginning of the experiment and lost weight more rapidly than cows fed C. Milk energy output, adjusted for tissue energy gain or loss, was 83·9 for cows fed C compared with 71·8 MJ/d for cows fed G (P < 0·001), during the period of tissue energy repletion (weeks 11–18). From week 18 to the end of lactation all cows from exp. 1 were fed silage ad lib. and 502 kg dry matter of concentrates. The total (305 d) difference in lactation response to grazing C compared with G was 931 1 (5657 1, C; 4726 1, G); this was a direct response during the experiment of 301 1 and a residual response of 630 1. In exp. 2, more organic matter (6·47, G; 8·01, C, kg/ d, P < 0·001), and more non-ammonia N (433, G; 575, C, g/d, P < 0·001) entered the duodenum of cows grazing C compared with G.
